Terms and Conditions

By choosing to enroll in services with me, you are agreeing to attend all scheduled appointments and keep in communication regarding schedule changes. I want to support you in staying accountable and making your healing a priority. If you are over 15 minutes late to your scheduled appointment and do not let me know ahead of time, your session will be forfeited, and you will be responsible for paying the full agreed-upon fee. If you miss 2 appointments in a row without notifying me beforehand, you will receive an electronic Warning of Termination letter via Simple Practice. If I do not hear back via phone call or email within 7 business days of sending this letter, services will be complete.
